Aregawi's junior career showcased his early potential. In 2018, at the age of 17, he participated in multiple major competitions. At the 2018 IAAF World U20 Championships in Tampere, Finland, he earned a bronze medal in the 10,000 metres event, finishing behind Rhonex Kipruto and Jacob Kiplimo with a time of 27:48.41.
The same year, he won a gold medal in the 3000 metres race at the Athletics at the 2018 African Youth Games held in Algiers, Algeria, clocking 7:50.98. Following this, he competed at the Athletics at the 2018 Summer Youth Olympics in Buenos Aires, Argentina, where he secured a silver medal in the combined 3000 metres event, accumulating 4 points. In November 2019, Aregawi won the Great Ethiopian Run, a 10 kilometres road race.

On June 8, he finished third in the Ethiopian trial for the 2020 Summer Olympics 10,000 metres, behind Selemon Barega and Yomif Kejelcha, which effectively secured his place at the delayed 2020 Tokyo Olympics. In his Olympic debut, Aregawi placed fourth in the 10,000 metres race with a time of 27:46.16, just missing a medal as Barega took gold.
Later that year, on December 31, Aregawi made history by setting a new world record in the 5 km run. He achieved this feat at the Cursa dels Nassos 5K in Barcelona, Spain, clocking an impressive 12:49. This time improved upon the previous mark held by Joshua Cheptegei by 2 seconds and gave Aregawi a significant 38-second margin of victory over the next competitor.
