One of his most iconic early film roles was as Neil Perry in the 1989 drama Dead Poets Society, where he co-starred with his childhood friend Ethan Hawke. Leonard's most famous onscreen role is that of Dr. James Wilson, the head of the Department of Oncology, in the Fox medical drama series House, which he starred in from 2004 to 2012. He had been invited to audition for both House and the CBS crime drama series Numb3rs. Although he found the Numb3rs script "kind of cool," he ultimately chose to audition for House, believing the character of Charlie in Numb3rs was in "too many scenes." He later admitted, "The less I work, the happier I am." Leonard believed that his House audition was not particularly strong, but his friendship with the show's executive producer Bryan Singer helped him secure the role. In 2007, Entertainment Weekly featured him on its "100 list" as "Dr. Underrated."

In 1991, Robert Sean Leonard co-founded the Malaparte theater company in New York alongside his childhood friend Ethan Hawke, as well as James Waterston, Steve Zahn, and Frank Whaley. The company was officially dissolved in 2000, partly due to its members' desire to devote more time to their families. Hawke later described the company's peak period as "pretty much the most thrilling period of [his] life."

Leonard maintains close friendships with several of his past co-stars and collaborators. He remains a close friend with his House co-star Hugh Laurie. He has also been friends since childhood with actor Ethan Hawke, with whom he co-starred in Dead Poets Society and Tape.
